Ośrodek „Pamięć i Przyszłość” został powołany do życia jako państwowa instytucja kultury w marcu 2007 roku przez Ministra Kultury i Dziedzictwa Narodowego Kazimierza Michała Ujazdowskiego.
Od początku 2009 roku do marca 2016 roku Ośrodek funkcjonował jako instytucja samorządowa miasta Wrocławia.
Od dnia 21 marca 2016 r. Ośrodek „Pamięć i Przyszłość jest instytucją kultury wspólnie prowadzoną przez z Ministerstwo Kultury i Dziedzictwa Narodowego i miasto Wrocław.
